Aged Care Services (ACS)

Finding yourself, or a loved one, needing more help at home can be really hard, and sometimes distressing. Our ACS team can work alongside you, and your family, to ensure that you are able to stay in your own home for as long as possible, maintaining dignity and independence.

There are many levels of support that we can offer which include government supported Home Care Packages (HCP) and the Commonwealth Home Support Program (CHSP).

Navigating your way through the maze that is aged care can be very daunting, but we are here to help you make sense of it all. We know that you would like to be able to choose how your Home Care Package will look, and we can help to make this happen.

Our Aged Care Services team will tailor your services to meet your individual needs. We provide holistic and personalised support and assistance to help you to remain living in your own home, maximising your independence and choice. We would like to help you to stay in your home for as long as you are able. We can take you shopping, out for coffee, take your dog for a walk, whatever you need.
As your needs change, we can adapt our services to accommodate them and to help you continue to live your life as you choose. We appreciate that this can be daunting so let us help you through the decision making process.

If you are a person 50 years or older who is homeless, or at risk of homelessness, then the Assistance with Care and Housing for the Aged Program (ACHA) can assist you to find the right accommodation for your needs, Including:

• Assisting you to find housing through the private market.
• Assisting you to apply for affordable social housing.
• Asssisting you if you are at risk of losing your present accommodation due to tenancy issues.
• Supporting you to find a Residential Aged Care facility.

Referral Pathway
You can be referred to the ACHA program through My Aged Care, Self, GP, other support networks and hospitals.

Footprints Home Care Packages are tailored to meet your individual needs, maximising your
independence and choice. We provide holistic and personalised support and assistance to help
you to remain living in your own home and avoid premature entry into residential care. We
offer a comprehensive variety of services to help you with your day-to-day activities.
To receive a Home Care Package you need to be assessed by an Aged Care Assessment Team
(ACAT) who will determine the package level to suit your care needs. Footprints will be happy to
assist you with this process.
Focusing on health and wellbeing, we provide experienced Case Management, Care
Coordination, Advocacy, Allied Health and Nursing and friendly reliable support staff. Your
package can be designed with you to help you access social opportunities and keep you linked
into the community. Together, we can develop a plan of support, which is consistent, reliable
and exible.

If you notice that you need a bit of extra help at home, then Footprints can help! To access our services you, contact Footprints and we can guide you through the My Aged Care process. Alternatively call My Aged Care on 1800 200 422, for an assessment and to be allocated an Aged Care number. Our services are subsidised by the Australian Government.

Eligibility
• You must be 65 years or older, or 50 years or older and identify as an Indigenous Australian or Torres Strait Islander person
• Still living in your home
• In need of help to allow you to continue to live independently

Our programs are designed to develop, maintain and support social interaction and independent living. People have the opportunity to develop friendships and make social connections.

The Centre Based Activities (CBA) program provides access to a range of activities for people residing in both south and north Brisbane. We provide a home cooked meal and transport as required.

What will it cost?
A nominal fee is charged per availability and / or day which includes morning tea, lunch and transport to and from the centre, if required. Fees may be negotiable if there is financial hardship.

Our funding
Our funding is provided by the Commonwealth Housing Support Program, Home Care Packages, Disability Packages, and Queensland Community Care Services.

Allied Health
Footprints offers a range of allied health services including a monthly podiatry service (appointments required) at the Light Street Centre. We also offer in home and centre based diversional therapy.
